I can think of 3 people off the top of my head, each with 3-7 years of stay, broken by periods of 1 to 3 months outside the country that required new VOA when they came back in. When it came time to sort out WP - for some 3 years ago, but one this past April (and after receiving their latest VOA in February) their entire history of stay in the current passport was used to calculate the fines.
Why would they look at years of previous visa extensions and forgoe the chance to apply penalties and extract cash because of a fresh VOA?
They wouldn't, and since they reintroduced WP enforcement in 2013/14, the entire length of stay since the first E Type visa in the passport is what they look at.
