You must have the Sangkat letter, and yes you'll need to pay - it is in the <$50 mark for sure, I had a family member do it and I got a bit of change out of a Grant to do this damn letter twice. Very easy. If you want to change the address on your license you also need a residency certificate issued by the commune office in the last three months.
New way of getting the Driving licence in Phnom Penh. Aeon mall II just opened a Week ago . Second floor. Staff very friendly. It was the same Price as the ministry of transport in Phnom Penh. So 10000 for the medical check and 30000 for the fee. So 40000 total. (10 dollars) . You need 2 photos. Old khmer licence and/or home Country licence (and One copy) . Original Passeport (and One copy) And proof of Residency (sangkat). For the payment they ask you to pay the 10000 cash and the 30000 with wing or Smart Luy ( the Smart office is not far from the main entrance on the ground floor. Fee 1000.) 45 minutes total. Great service.
Re: translation I really don't know, but I'm guessing you're going to need an official translation of a non-English/French license; probably can be done at Pyramid or other?

Recently? Not sure if things changed - I got mine only 3 weeks ago; but also I was strangely applying for a new license rather than renewal. I've had a license for 11 years, but let the last one lapse about 2 months beyond the expiry and they put me in the category of simply getting a new license. Not sure if that is the difference. Anyhow, the letter shared above is the format I'd suggest; I had another "Work permit" purpose residency letter rejected by the jobsworth guy there. Shit place - Incompetent Authorities in action in plain sight.

First time applicants need a Sangkat letter ( Proof of residency ) , or if you want to change your address on your existing licence .
For just a licence renewal , no residential letter is required .

So I went to renew my expired 1 year driving license at Aeon II last Saturday and had the painful surprise to learn that I had to pay a 24$ fine (without receiving any receipt).
i know fines had increased since May 1st but I didn't expect such a hike and especially not receiving any receipt for that so called fine raised my eyebrow a little bit...
