Could anyone provide me with an English translation of the Khmer text in the column of vaccinations in the Khmer immunisation book that's given when your child is born. I'm currently in the U.K. and my local GP wants to know all the dates and inoculations my daughter has had before they assess what she needs over here.
I understand the abbreviations eg. BCG, OPV etc but just in case there was any extra info that i am missing.

Wouldn't it be sensible for you to post a picture of what you want translated so that people who don't have those immunisation booklets memorized can help you?

Vaccine Type || Date Taken || Health Center(location)
Basically the khmer is either a khmerization of the disease (tetanus, vaccine) or the khmer term for the disease
JE, japanese encephalitis = irritated brain disease
Whooping cough = chicken cough
etc.
There is a 'various vaccines' box at the bottom of the first photo. First box from the bottom is for measles (and JE).
The second picture is pretty much the same as the top. It's headers:
