Two questions, can anybody enlighten me, please:
1. Will they issue an ordinary visa at Baan Pak Kad crossing, going to Pailin from Thailand?
2. Bonus question: Any chance they will let me drive my Thai registered car all the way to Pailin?

1. Yes. Make sure you have the correct change in USD, don't fall for any of their scams, stand your ground, $35 ONLY.

1. Already answered and yes. I'd further comment that this crossing was a breeze for myself the two times I"ve used it (I was also travelling with a Khmer woman whose sister crosses the border multiple times a week and knows the guards - and was present - so I'm sure that helped).
2. I was told that if we had a car we could drive into Cambodia and could be issued a "permit" that would allow us to drive within the province. But if we went to the next province we needed another permit. And supposedly you could get a permit for the whole country in PP (exactly how you do that if your driving into the country is beyond me). But I'd take this with a grain of salt as it was an offhand question that was translated to a immigration guy late in the afternoon.
The other way around (I also asked at Thai border) guy said that I could get insurance and then I'd be given some type of temporary permit that would allow me to drive into Thailand from Cambodia.
I have NOT actually done either.

Thai cars entering Cambodia from Ban Pakkard should be allowed to drive to Pailin no problems, but may be told not to go further unless they have a permit from Phnom Penh. You see dozens of Thai cars on the Cambodian side all the way to Pailin; many of these are driven by Thai locals going to the casino or by contractors/businessmen from Chantaburi with business in Pailin. Note that only Ban Pakkard allows entry for a Thai car - not Ban Laem (although re-entering Thailand there is OK).
You should be OK driving past Pailin even without a permit BUT you need to at least pretend you won't leave the province, otherwise Cambodian customs could refuse you or become upset. You may or may not need to return through the same crossing.
Cambodian cars can also enter Thailand here but technically aren't supposed to leave Chantaburi province either. In practice, you can go further without issue but again, may or may not be required to re-enter Cambodia at the same crossing.
Phuketrichard never mentions Ban Pakkard as an entry point for vehicles, but in fact, it should be OK. However, O'Smach is the best and Koh Kong is also OK - relatively few people/cars enter at Ban Pakkard except those going to the casino or nearby places.
