Thailand and US, both parents need be there till they are (under 15 for thai and from 15-20 need both parents authorization letter, under 16 for usa) and if both cant, one can submit a notarized authorization letter.
Just spoke with a friend who recently got their kid his pp, his wife and kid went to the PP office without her husband, took kids birth cert, family book and her ID card...no requirement for the father to be present.
There's a requirement to get parts of some PP applications signed with a witness (accountant, lawyer, doctor, etc...), which is of course completely fool-proof.
Some countries, I believe the USA is one, require you to have documents to prove that a child traveling without both parents is not being abducted/ trafficked etc.
