My experience is to use agents unless the paperwork is extremely easy.
I have avoided medicals, headaches, and sometimes when you actually can't get it I have turned around and gotten it from an agent.
Sometimes you need to go to a different country and get an agent. For example it is very easy to get a tourist visa to China here through Lucky, but if you are going to process a Z visa to China you can nip across to HCMC and an agent can take care of everything without your medical or you filling out anything.
I also know a guy that has an Asian wife and if you leave your country of citizenship it is sometimes much easier to get the visa you need to Europe.
You have to know where the good agents are.
