Find out in your Sangkat, where you have to go for the vaccine card. There, they have big lists hanging with cards they already have to hand out. IF you're not on the list, take care to bring the details paper, passport and some "proof" why you need the vaccine card urgently (flight tickets, for example). They'll make pictures of the paperwork, sent that to the MoH and a week later, you can pick up the vaccine card. I did do that path and it works.
I have already received my card. And some of the other people who haven't received cards didn't get military jabbed.

This has been going on for weeks - it's clearly lost and won't be arriving now, it's just there has been no urgency for them to actually have the physical card. Until maybe now.
