Urgently needed: decent doctor for AIDS treatment
A friend of mine has AIDS. She was only diagnosed with the HIV-infection about two months ago, after she got severe symptoms, and has a high viral load. She is on ARV therapy now, but suffers from severe side-effects. She has little confidence in her present doctor, as he is a bad listener and doesn't seem to care much. I find it strange that she still has no clue when the next new blood test should be done, since I'm informed that each HIV/AIDS patient should be regularly tested, in order to determine the viral load and if necessary, adjust the medication. Could anybody recommend a good and affordable doctor?

The second poster today asking about HIV.
I don't think we have any doctors that are posters, but hopefully someone will have a suggestion or two. Is your friend Cambodian, and is she paying the expenses by herself or are you helping? (I only ask as that could affect the suggestions for care.)

Russian hospital is the place to sort this out.
The patient should be tested every three months to check viral load. The specific ARV might need to be changed if side effects are too severe, but once the medication is working, the viral load will likely reduce to undetectable levels.
I have some insight into this matter as I have been providing a little support to an HIV positive family in Phnom Penh for the last few years.

She has too little money of her own. She used to be a teacher at a well-known primary school, but was sacked for being HIV-positive. (The school seems to be afraid that parents won't send their children there if a teacher is known to be HIV-positive.) She is now making a few dollars per day with private tutoring. I'm supporting her a little with paying for the doctor's consultations (ARV drugs are free). Right now things seem to be going very wrong, and I hope I'll be able to save her life.
Spend some money. Money will get you knowledge/experience. And knowledge/experience is what you need. Not just a person with a certificate on the wall telling you he or she is a doctor.
Having said that, I think RIHAC offer free services for Cambodians when it comes to HIV / AIDS, and maybe that's the best you can get for free,

She has been to the Russian hospital but was told they don't accept any new patients for the time being as they already have more patients than they have time for. She is Cambodian and she is poor. I don't want to engage in a discussion of how poor she exactly is, who is paying for what, or what I may or may not contribute. I'm simply trying to save a life, and all I want is a recommendation of a decent doctor. Royal Ratanak etc. is no option as it is unaffordable.
I found Khema Clinic in Toul Kork to be good. Expensive by Cambodian standards, but good, and cheap compared to Royal Phnom Penh. But in my opinion saving a life when HIV-AIDS is involved requires knowledge, and that doesn't come for free here.

Thanks, that is pretty good information. I will pass that on to her. Of course, money shouldn't be a consideration, but as she is poor and makes only a few dollars per day, it is.

Thanks for all the info, I will pass it on to her. Of course, I don't expect anything for free. She tells me that state hospitals and NGOs provide free medication, but the regular consultations and test have to be paid whereas private clinics charge patients for both consultations AND medication. As the Russian hospital doesn't accept new patients, I wonder if it is possible for her to get the consultations and tests at some private clinic, and then go to a state hospital with the prescription to get free ARV drugs. It would be unfair if some AIDS patients get the drugs for free while other, equally poor patients have to pay for them, just because of a capacity problem at state hospitals.

Yes sometimes life isn't fair.
I just wonder what you are really looking for. if she's looking for free services for Cambodians you don't seem to be the right person to look for it, and this forum aiming for expats in Cambodia is maybe not the right place to ask for it.
There's several NGO's in Phnom Penh offering services and support to to Cambodian AIDS patients, but I don't know about the quality of their service, and I guess most expats here are not familiar with their level of service too. Maybe put the ball back in her court?

I'm just trying to help a little, both financially and with information, and what she does with the information is up to herself. Thanks!

Our organization has come a long way since 2002, when AUA first started as an informal peer support network for people accessing ARV (antiretrovirals) at the Khmer Soviet Friendship Hospital in Phnom Penh.
