Speaking of newbie American travelers, I just met one in the visa line at Phnom Penh airport who probably wasn't all that experienced. He was grumbling that he had just flown in from Siem Reap and was being charged for another visa. I had no idea what he meant, until he explained that he flew from Siem Reap to Phnom Penh on Bangkok Airways with a connection in Bangkok. He said that's how his travel agent back home had booked it.

OMG, epic - his T-visa - probably all of 3 days old - became invalid the moment he went through security for his international flight out of the country.
