I saw a documentary about some lawyer who had AIDS and it tracked his life during a trial against his former employees. You gradually saw him deteriorate during the trial. Set in Philidephia I think.
Anyway. I thought he was going to die but then I saw him marooned on a desert island with Willson and a FedEx box.

So, your mind interpreted that as only sometimes is the sex he has 'safe', where as it is equally likely (actually more likely) that it was intended to communicate that sometimes they'll have sex and when they do it will be safe sex. Or, maybe it won't be, and why would that matter when the other person is HIV+ too? That's why those who are HIV+ seek out others who are HIV+. Where have you been living?
